Child Custody and Visitation

North Penn Legal services has compiled the following Pro Se packets designed to assist parties in filing for child custody and visitation.  These packets are available to print or pick up in the Court Administrator's office.

Please note that Neither the staff in the Court Administrator's Office nor the staff in any office of the Courthouse will be able to give you legal advice or help you fill out/complete the forms included in these packets.  The information in the packets is not a substitute for professional legal advice.  The Court assumes no responsibility and accepts no liability for actions taken by users of these documents, including reliance on their contents.  If you have any questions concerning the packets you can review the Child Custody and Visitation Frequently Asked Questions or contact North Penn Legal Services at (877) 953-4250.

Please do not change these documents or the court will not accept them.

Note:   Please contact the Prothonotary's office to inquire about filing fees for each of these packets.  The individual filing the packet is responsible for  paying the necessary filing fees.

Important notes before filing for custody - there are many myths and rumors about child custody, visitation, and support.  This document will help you narrow down your rights and responsibilities before you file for custody, visitation and/or support.

Custody Agreement - if both parties agree to the terms of custody and visitation you can sign this agreement packet in front of a Notary, obtain the Judge's signature, and file it in the Prothonotary’s office.
 
Custody Contested- if both parties cannot come to an agreement this packet can be signed by the Judge, and filed in the Prothonotary's office.  The person filing the order will be responsible for serving the opposing party.
 
Custody Modification - Agreement - if there is an existing custody/visitation order in place this packet can be used to modify that order.  The person filing the order will be responsible for serving the opposing party.
 
Custody Modification - Contested - if there is an existing custody/visitation order in place this packet can be used to modify that order.  The person filing the order will be responsible for serving the opposing party.
 
Contempt of Custody/Visitation- if the opposing party is willfully disobeying the existing order this packet can be used to file a contempt order with the Court of Common Pleas.  The person filing the order will be responsible for serving the opposing party.
 
Contempt of Spousal Support/Alimony - if the opposing party is willfully disobeying the existing order this packet can be used to file a contempt order with the Court of Common Pleas.  The person filing the order will be responsible for serving the opposing party.
 
Relocation form - if you want to relocate with your children out of the jurisdiction of Tioga County
 
IFP Form- Use this form if you are believe you are unable to pay for the filling fees associated with filling your custody or visitation action.

 

 PRO SE PAPERWORK - new administrative order
