LIST OF CUSTODY SELF-HELP PACKAGES AND FORMS

PLEASE USE THIS LIST TO SELECT THE SELF-HELP PACKAGE OR FORM THAT IS
APPROPRIATE FOR YOUR SITUATION

• NEW CASE (WITH AN AGREEMENT): If you and the other parent have an agreement on how custody should be handled, but there is no custody order in effect yet, you will need the 'NEW CUSTODY AGREEMENT SELF-HELP PACKAGE.'

• NEW CASE (WITHOUT AGREEMENT): If you want to file a new custody case without an agreement and no custody order is in effect yet, you will need the 'CUSTODY COMPLAINT SELF-HELP PACKAGE.'

• MODIFICATION OF CUSTODY (WITH AGREEMENT): If you and the other parent have agreed to modify an existing custody order, you will need the 'CUSTODY MODIFICATION AGREEMENT SELF-HELP PACKAGE.'

• MODIFICATION OF CUSTODY (WITHOUT AGREEMENT): If there is already a custody order in effect and you want to modify it, but the other parent does not agree, you will need the 'PETITION TO MODIFY CUSTODY SELF-HELP PACKAGE.'

• RELOCATION: If there is already a custody order in effect and you want to relocate with your child(ren), you will need the 'RELOCATION SELF-HELP PACKAGE.'

• CONTEMPT: If there is already a custody order in effect and the other parent has violated the existing custody order, you will need the 'CONTEMPT PETITION SELF-HELP PACKAGE.'

• REQUEST WAIVER OF FILING FEES: If you cannot afford to pay the filing fees for a custody case and you need to ask the court to waive the filing fees, you will need the 'IFP PETITION SELF-HELP PACKAGE.'

• CRIMINAL RECORD/ABUSE HISTORY VERIFICATION FORM: If the other party has filed a complaint or petition for custody against you and you have been ordered to complete a criminal record/abuse history verification form, you will need the 'CRIMINAL RECORD/ABUSE HISTORY VERIFICATION FORM.'

• SELF-REPRESENTATION FORM: If the other party has filed a complaint or petition for custody against you and you do not have an attorney, and will be representing yourself in court, you will need a 'PRAECIPE FOR SELF-REPRESENTATION FORM.'

• UPDATE CONTACT INFORMATION: If you represent yourself and your address has changed,you will need a copy of the 'PRAECIPE TO UPDATE CONTACT INFORMATION FORM.'

• CUSTODY SUPERVISION AFFIDAVIT: If your case involves an order for supervised visitation,you will need the supervisor(s) to sign the 'CUSTODY SUPERVISION AFFIDAVIT.'

• CONTINUANCE REQUEST: If you are asking the court to reschedule a previously-scheduled court proceeding in a custody case, you will the 'CUSTODY CONTINUANCE SELF-HELP PACKAGE.'
